Listed below are the health & dental insurance plans available to employees effective 7/1/12. Which plan is available to you depends on which union you are with and your date of hire.
All the forms and information pertaining to the new plan year (7/1/12 - 6/30/13) are on this Open Enrollment page. Do not use the forms from the Health Insurance page, those forms are only good until 5/31/2012. Also, please ensure that you look at the right information and complete the correct forms that correspond with your union and date of hire.

Health Insurance Buyback Program Rules & Forms
This program offers a cash incentive to eligible City of Manchester employees who waive thier right to enroll in the City's Health Insurance plan. Cash amounts & rules are different depending on which union you are with.

New High Deductible Plan
We are pleased to offer you a new High Deductible Health Plan this year, the Regional High Deductible Health Plan. It offers the same plan design you currently enjoy, with the added benefit of potentially lower out of pocket costs to you when you recieve services. See the chart below to assist you in determining which HDHP may be best for you and your family.

- Consists of: Contracted providers throughout the 6 New England States (NH, MA, ME, VT, RI & CT).
- Choose if: If you and your family members reside in New England year round.
- Reason for choosing: This will give you the opportunity to lower your costs because the network under this plan holds contracts with some hospitals at a far deeper discount than the National HDHP. For example, the charge for an MRI may cost you $1,100 under the National HDHP, where it would cost $800 under the Regional plan.
- Important: You must designate a Primary Care Provider

- Consists of: Contracted providers throughout all 50 states.
- Choose if: If you and/or any of your family members live outside the New England area for an extended period of time. For example - you live in Florida for 4 months of the year or if you have a child in college outside the New England area.

Health Savings Account Forms & Information
You must be enrolled in the Lumenous High Deductible Health Plan in order to have a Health Savings Account. The deductible for the plan and the amount the City will contribute towards your deductible is listed below:
- Single Person HDHP deductible is $2,000 and the City will contribute $1,500
- 2 or more Person HDHP deductible is $4,000 and the City will contribute $3,000
A Health Savings Account is a tax-exempt savings account used for the express purpose of paying qualified medical expenses. As long as funds are saved and spent for qualified medical expenses, all contributions (including those made by the City) and withdrawals remain untaxed. Unlike the flexible spending account, the funds in an HSA are not subject to a "use it or lose it" policy. Anything you don't spend one year, carries over to the next year.
You cannot have both a Flexible Spending Account and a Health Savings Account. If you currently have a Flexible Spending Account it will be converted to a "Limited Use" Flexible Spending account for vision and dental only effective 7/1/2012 and you cannot re-enroll in the Flexible Spending Account effective 1/1/2013.
The City utilizes Citizens Bank for the HSA Account, you must register into Citizens bank to open up your HSA Account. Instructions for how to open your Citizens Bank Health Savings Account is listed below. Once your account is open and you have your account number, please contact Sue or Jane your benefits coordinators at benefitscoordinator@manchesternh.gov and tell us your account number. This is necessary in order for us to deposit the City Contribution amounts into your HSA account.
