High Deductible Health Plan-The medical plan
- Preventative care is paid at 100%
- Non-preventive care (including prescriptions) is paid by the member until the deductible is met
- $2,000 deductible for an individual plan
- $4,000 deductible for a two-person or family plan
- In-network medical care (covered by the plan) is paid at 100% once deductible is met
Medical Plan Summary of Benefits
Health Savings Account(HSA)-A bank account to pay for your deductible
- City contributions to (active) employee HSA's to help pay deductibles
- $1,500 per plan year for single plan ($1,300 for PDSS Union Members)
- $3,000 per plan year for two-person and family plan ($2,600 for PDSS Union Members)
- City contributions are pro-rated for mid-year enrollments
- Remaining funds in your HSA roll over into next plan year
