The City of Manchester offers the UNUM Critical Illness Insurance plan to all employees (working 30 hours or more per week) and their dependents.
Critical Illness Insurance provides financial support by offering a lump sum benefit payment directly to you upon diagnosis of certain serious illnesses.
This payment can be used as needed, such as for out-of-pocket medical expenses like co-pays and deductibles. The coverage is valuable because it can be used more than once, even after receiving a payout for one illness, and covers various conditions including heart attack, stroke, cancer, major organ failure, and progressive diseases like ALS and Parkinson's disease.
Additionally, health screenings are covered annually, including tests for cancer, cardiovascular function, cholesterol, and diabetes.
Coverage is available for you, your spouse, and your children, with no medical questions if applied for during enrollment. All children dependents will be covered automatically. The amount of spousal coverage is equal to half of what you choose for your own coverage. Ex: You choose to take coverage for $10,000, then the spouse's amount is $5,000.
Insurance Amounts:
- Empoyee Coverage: $10,000, $20,000, or $30,000
- Spouse Coverage: 1/2 of the employee's coverage amount ($5,000, $10,000, or $15,000 respectively)
- Child(ren) Coverage: 50% of Employee Coverage for each child (up to age 26)
This benefit is voluntarily enrolled, managed, and paid by employees, who are accountable for covering 100% of the associated expenses. The costs will be deducted in payroll. To file a claim for any of the voluntary benefits with UNUM, please call 1-800-635-5597.
