Healthy Start Home Visiting Program
The Healthy Start Home Visiting Program is a partnership between the City of Manchester Health Department and Amoskeag Health that provides expecting mothers with home visits by a Registered Nurse to help connect families to resources, aid in achieving the best health possible for themselves, their baby and family, and reach health and life goals. Lactation services are also available.
Maternal health awareness is the first step toward saving lives, building stronger families, and transforming communities. Maternal health is a community effort. Healthy Start aims to limit barrier to accessing care by seeing families in their homes and collaborating with community health workers.
Referral Requirements
Enrollment is arranged through Amoskeag Health Prenatal Care Coordinators and their care team providers. All prenatal patients who reside in Manchester are eligible.
