Authorization and Certificate
I authorize the City of Manchester, NH, at the time of my application for employment or during the course of employment, to obtain information from any source as to my education, experience, character, financial or credit record as it relates to the position for which I am being considered, or in which I may be employed. I certify that my statements in the application are true, complete and correct to the best of my knowledge and belief. I understand any falsification or omission of information may bar me from the examination, may result in the removal of my name from the eligible list, or if I have been appointed, may cause my dismissal. I also agree that all statements made on this application may be investigated.

Keep Us Informed
Please tell us if you change your address or phone number. If we cannot locate you, you may miss a chance at a job. You can apply for several jobs with one application. Most qualifying experience is paid work experience. Volunteer experience should be clearly noted as non-paid.

Background Investigation
If hired, your background will be investigated. Be honest. Show on your application every law violation (except parking tickets) for which you were found guilty or to which you pleaded no contest. If you do not remember your record, contact the appropriate law enforcement agency. Law violations are reviewed in relation to the job for which your are applying.

Physical Exam
If offered a position, you may have to pass a physical examination. This examination is performed by City doctors, and there is no cost to you.

Immigration Reform Act
The Immigration Reform Act of 1986 requires both identity and eligibility for employment proof within three days of employment.
