Health Insurance Opt-Out Incentive Program
The Health Insurance Opt-Out Incentive Program offers a cash incentive to eligible City of Manchester Employees who waive their enrollment into the City's health insurance plan.
To enroll in this opt-out program, please download the Health Insurance Attestation Form here, fill it out and send it to your payclerk. Remember, employees must submit this attestation form annually.
