Case Management, Counseling, Information and Referral

 

Eldercare Locator

Eldercare Locator is a nationwide, directory assistance service designed to help older persons and their caregivers locate local support resources for aging Americans.

 

NH Division of Health and Human Services - Bureau of Elderly & Adult Services

The Bureau provides services under the Adult In-Home Support, Alternate Care and Protection programs.  These offer a broad range of services for those who meet financial eligibility guidelines.

 

ServiceLink

ServiceLink offers supportive information and referral for services and programs offered in the Manchester and Hillsborough area.

 

Housing

 

Types of Housing

Assisted Living - A place operated on a 24-hr basis to provide residential accommodations, personal care, and recreational services to individuals whose health and safety require continued supervision.

Congregate Housing - Independent living with personal care and support services provided including meals, housekeeping services and congregate activities.

Subsidized Housing - The tenant pays no more than 30% of his/her adjusted income for rental cost. 

Nursing/Rehab Centers are for people who have difficulty caring for themselves.  There are two types of care provided: Intermediate care for those who don’t require constant care, but cannot live independently and Skilled Nursing care for those who need extensive care, but not hospitalization.

Residential Care provides help with daily chores, but allows individuals to come and go as they please.

Retirement Communities are a small, self-contained complex that provides minimal in-unit services to the residents while offering a sense of community spirit, convenience and security.

Manchester Housing and Redevelopment Authority

MHRA provides Section 8 and Public Housing and Congregate Services for low-income eligible families and seniors in hi-rises and scattered sites throughout the City.

 

Southern NH Services

SNHS has connections to two subsidized housing sites in the City.

 

New Hampshire Catholic Charities

Catholic Charities is affiliated with two nursing homes in the City as well as elderly apartments that provide quality independent retirement living for seniors.

 

Health

 

Elliot Hospital

The ElliotHospital is a full service hospital providing a full range of patient services.  The ElliotHospital also provides Home Health Care Services through the VNA and is affiliated with the ElliotSeniorHealthCenter.

 

Catholic Medical Center

CMC is a full-service health care facility that provides a full range of inpatient and outpatient services.   Prime Time (informational and educational center for seniors and their families) as well as the Great Day Program and the Parish Nurse Program both of which offer health-counseling, advice, wellness programs and health screenings.

 

Child and Family Services

CFS Home Care Program strives to maintain quality of life for seniors, at risk ill or disabled individuals.  Caregivers assist residents who desire to remain on their own by carrying out light housekeeping duties, doing laundry and planning, shopping for and preparing meals.

 

Mental Health Center of Greater Manchester

The Senior Counseling Services provide community services to persons 60 and over who are open to receiving our services.  Transportation is provided for those able to come to the center.  Clinicians do outreach in homes, hospitals or nursing homes.  Direct services include individual and group therapy as well as diagnostic and psychiatric evaluations.  Indirect services include psychiatric consultation and educational workshops on aging issues for seniors and their families.

 

Visiting Nurse Association of Manchester & Southern NH, Inc

Services offered by the VNA include, but are not limited to, skilled nursing, physical, occupational and speech therapy.  Some services require a physician’s order.

 

St Joseph Community Services, Inc

The WmBCashinSeniorActivityCenter is one of three meal sites in the City that is managed by SJCS.  The program provides home delivered meals (Meals on Wheels) to homebound seniors and also provides congregate meals Monday through Friday.  Lunch at our location is served at 11:30am.  Please call 668-0254 a week ahead to reserve a meal.  For information about services and other meal sites please click on the above link.

 

Adult Day Programs

The Elliot Adult Day Program is affiliated with the Elliot Hospital and provides a broad range of social and nursing services that support aging adults and their families/caregivers.

 

Easter Seals offers four different adult day programs to meet the needs of frail seniors.

 

Emergency Response Systems

Emergency Response Systems ensure that older adults living at home get quick assistance whenever it is needed 24 hours a day.  There are numerous companies and/or agencies that offer this service.  Please contact our office for further information.

 

Home Care Agencies

Home care agencies offer non-medical services such as transportation, assistance with grocery shopping, friendly visits and reassurance calls.  There are numerous companies and/or agencies that offer this service.  Please contact our office for further information.

 

Income

 

City of Manchester Welfare Department

 

Elliot Hospital- Charitable Care Program

The Charitable Care Program ensures that all individuals are able to receive medically necessary care at the ElliotHospital regardless of their ability to pay.  The Elliot Hospital offers other programs that offer patients financial assistance.  For further information, or to apply call 663-2571.

 

Uncompensated Care at CMC

Uncompensated Care at CMC is a mission-based program for reduced or free care.  Apply at CMC Business Office or contact the Department of Social Services at 663-8746.  CMC also participates in the New Hamsphire Health Access Network and the Hospital Access Plus programs.

 

NH Division of Health and Human Services - Division of Human Services

The Medicaid, Food Stamps, Old Age Assistance, Specified Low-Income Medicare Beneficiary (SLMB), Qualified Medicare Beneficiary (QMB) and Qualified Disabled Working Individuals (QDWI) programs are administered by the Division to income eligible individuals.

 

New Hampshire Health Access Network

The NH Health Access Network is a group of hospitals, doctors and other health care providers in NH that work together to help children and adults when they can't afford the health care they need.

 

PocketBook Therapy

PocketBook Therapy is an innovative program that helps seniors take advantage of existing prescription assistance programs for medications.  Volunteers also assist seniors by finding subsidies for other monthly expenses such as rent, utilities, etc.  Sites with trained volunteers are the Manchester Housing and RedevelopmentAuthority, the Wm B Cashin Senior Activity Center, Southern NH Services and the First Congregational Church.

 

NH State Health Insurance Counceling and Assistance Program (SHIP)

SHIP volunteers offer Medicare recipients counseling

 

Social Security Administration

SSA provides monthly income protection for workers and their families in the case of retirement, disability, survivorship, Medicare and Supplemental Security Income (SSI).

 

Southern New Hampshire Services, Inc.

The Commodity Supplement Food (CFSP), Fuel and Electric Assistance, Weatherization, Fixit, Senior Energy Assistance (SEAS) are a few of the programs that offer assistance with daily living expenses for income eligible families and individuals.

New Hampshire Legal Assistance

The senior Citizens Law Project (SCLP) provides free legal advice, counseling and representation in civil cases relating to housing, health care, nursing care and public benefits.  There are no asset and/or income eligibility limits for clients over 60.

 

New Hampshire Bar Association

The NH Bar Association (603-229-0002) offers a Lawyer Referral Service, a Reduced-Fee Referral Program, a Pro-Bono Referral Program and a Legal Advice and ReferralCenter (1-800-639-5290) for low-income individuals.

 

Recreation

 

The WmBCashinSeniorActivityCenter offers a variety of activities for seniors over 60.  Please see our list of activities and/or our calendar for details.

 

Transportation

 

CMC Care Van

The Care Van offers round trip transportation to CMC, DartmouthCommons, lab sites, doctors, and physical therapy.  Please call 663-6713 for information.

 

Elliot Shuttle

The Elliot Shuttle is a free transportation service, for citizens over the age of 55, to and from the ElliotHospital and its outpatient departments, ElliotSeniorHealthCenter and physicians offices.  Please call 663-5609 for information.

 

CareGivers, Inc.

The Caregivers provide outreach through volunteer services such as transportation, chore services, friendly visiting and respite care.

 

Granite State Independent Living

GSIL is a statewide non-profit organization that recognizes the fact that all of us will need some type of support in the course of our lives.  They offer the tools and resources so that individuals can participate as fully as they choose in their lives, families and communities.  GSIL offers Deaf Services, Information and Referral, Peer Support, Service Coordination, NH Phone Link, Transportation Services, Access Modification Services and Accessible Housing.

 

Senior Companion Program

Senior Companions visit other seniors to provide companionship and friendly visits.  They encourage emotional and physical independence.  They may also provide respite for caregivers.  The Senior Services Department, Manchester Housing and Redevelopment Authority and the VA Medical Center are all stations for this program.

 

MTA's Step-Saver

Step-Saver provides door to door van transportation service for disabled people 18 and older for a minimal fee.  An application must be completed prior to the rendering of services.  Call 623-8801 to receive an application.

 

Special Transit Service, Inc.

STS provides transportation to seniors within the Greater Manchester area.  Call 668-8603 x6685 to make arrangements.
